GPS is a group practice which specializes in the holistic care of sub-acute, skilled nursing and long-term stay patients. GPS provides high-quality clinical, administrative, and practice management oversight within Genesis facilities. Quality of life is always our focus, but getting people back to a healthier state is our promise and what motivates us. We’re handling hospitalist acuity in a sub-acute environment; often completing the plan of care initiated in the hospital. As part of a care team, we’re addressing complex medical comorbidities — an exciting challenge and unique opportunity to impact change. As part of a pioneering team, you will most certainly play an integral role in changing what we know about effective healthcare delivery and restorative patient care.
What’s different about the Genesis way?
Genesis is unique in that we are the only national operator of skilled nursing facilities that has a significant staff of physicians and advance practice providers whose primary jobs are inside our facility. At GPS, we visit the patients in their homes (our nursing facilities) vs. making them come to us. We bring primary care directly to the patient. In addition, Genesis has formed its own accountable care organization (ACO) through the Medicare Shared Savings Program. This means we have full accountability for population health, quality, outcomes and costs.
For short stay patients, our focus is on vitality. Our clinicians are leading the diagnostic and therapeutic decision making process, so there’s a lot of clinical autonomy. Our goal is to successfully discharge patients home, but during this time, we are also working to prevent additional morbidities and rehospitalization. By helping people through what may be the toughest life transition they’ve faced, we’re a crucial link to future wellness.
For long stay patients, our focus is providing dignity and quality of life in a residential setting. For our geriatric patients, we are responsible for doing chronic disease management, advance care planning, and palliative care.

About Our Physicians
At GPS, our skilled and attending Physicians are our true leaders, and ensure the highest level of care, successful patient outcomes and compassion that we are recognized for.
Healthcare has changed, and the interdisciplinary team environment is here to stay. At GPS, the physicians who have the most success are leaders that are able to take charge, orchestrate care, and embrace the team aspect of caring for our patients and residents. If you’re an experienced physician — MD or DO — who can excel in a fast-paced, interdisciplinary environment, we encourage you to join our team.
There is even the potential for the attending Physician to become a center’s Medical Director. Due to the increasing accountability for outcomes, the Medical Director role at Genesis is expanding both in importance and responsibility. We expect and reward effective and proactive leadership, and the Medical Director is truly partnering with the center’s Executive Director, Nurse Executive, and Director of Rehabilitation. It’s our vision that every center will be led by these individuals as an interprofessional leadership team, each having fully shared accountability for overall center performance.
Post-Acute and Long Term Care
At our skilled Post-Acute and LTC facilities, GPS Physicians and Advanced Practice Providers often oversee patients that require AIM (Advanced Illness Management) and Transitional Care Management.
Many of our patients have received high level hospital care prior to admission and continue to have high-acuity needs during their stay. Our Physicians and Advanced Practice Providers practice acute, hospital-level specialty medicine and care in our unique skilled nursing home environment. The average length of stay for skilled patients coming in for rehabilitation in our facilities is 3 weeks.
Our PowerBack model is a new and exciting personalized care program that utilizes the latest equipment and aggressive therapy plans to get our patients home as soon as possible.
PowerBack offers an incredible opportunity for Physicians and Advanced Practice Providers to be part of a dynamic model known for shorter post-hospital patient recovery periods, fewer post-discharge setbacks and returning patients back home as quickly and safely as possible.
